Change your email address because your name changed

This form is only used in order to make your email address reflect an official name change. If you instead want to create an additional email address to point to your existing email address, please resort to the Get an Email Alias form.

If you legally change your name, sometimes you need to change your official UNT email address without creating an alias. Once your official UNT email address changes, you will need to change your login name on every device you check UNT email with (only on the devices that use your email address instead of your EUID as the username). Listed below are the methods to go about changing your official name with UNT.

First name change:

  1. Log in to
  2. Under the Human Resources section, select the Personal Information section on the left side
  3. Select Name Change Request
  4. After selecting a new first name, if no proof is requested by HR, please use the form below to let us know you need a new email address.

Surname change:

  1. Contact UNT HR. Once they have confirmed the name change, please let us know using the form below.
